In this latest BCLA podcast, hosts Karin Brown-Balvert and Fakhruddin Barodawala chat with Helen Liaw. She shared insights from the recent BCLA ASIA conference held in Chengdu, China. The BCLA ASIA was held in collaboration with the 2024 West China International Myopia Control and Eye Care Symposium from March 8th -9th, 2024. Helen shares the updates about the myopia practice's in the different regions which were presented as research presentations during the conference.

The first podcast of the year 2024 where hosts Karin and Fakhruddin discuss about the contact lens industry with the BCLA President, Rakesh Kapoor. Rakesh shares his thoughts on the CL industry and the role of practitioners to make patients aware that they can wear contact lenses. He related this discussion to his presidential address and also the Editorial which was published in CLAE in the February 2024 edition.

She also dives deep into the evidence of the different management options available for our patients suffering from dry eyes and provides insight into how this evidence-based approach can be used in clinical practice.

Dr. Santodomingo briefly summarised the findings of his presented research, he also gave an overview of the other papers which were presented during the session which he moderated. He was also pleased to share with us the newer findings of his research on the efficacy of orthokeratology lens wear in reducing the axial elongation of the eye in children.

There is anecdotal evidence that we now see more patients having complaints of dry eyes - specifically post-pandemic. This could attribute to a lot of factors including the time spent on digital devices in recent times. In this podcast, BCLA Medal Award Winner for 2023, Prof Eric Papas shares his research experiences with Karin and Fakhruddin from the BCLA Digital Learning Committee about the incidence and prevalence of dry eyes. He also provides clinical tips on how this research translates into clinical practice and how can a clinician better diagnose patients with dry eyes. The lecture is part of the 2023 BCLA Clinical Conference and Exhibition in Manchester. Depending on when you listen to this there is still time to register for the conference and learn more from Prof Eric Papas' Medal Lecture where he will discuss his spectrum of work including the development of silicon hydrogel contact lenses, fitting contact lenses for patients with albinism, ocular surface response to contact lens wear and many more.

*Eric Papas is Emeritus Professor in the School of Optometry and Vision Science, University of New South Wales. He holds degrees in physics and optometry and has spent much of his career in research related to the ocular surface and contact lenses, with periods spent working in clinical practice, industry and academia.

The Feb 2023 issue of Contact Lens and Anterior Eye features a review to mark the 75th anniversary of the introduction of the original corneal contact lens of Tuohy in 1948. Patients wearing Tuohy lenses were able to achieve a much longer wearing time compared to those wearing sealed scleral lenses since they eliminated corneal epithelial oedema, a frequent problem which necessitated removal of lenses after a few hours of wear due to cloudy vision. Only some of Tuohy lens wearers exhibited an adverse corneal response such as fluorescein staining after 12, or more, hours of wear. The clinical success of the Tuohy lens and the fact that its fitting procedure was simpler and quicker than that for scleral lenses encouraged optometrists to commence contact lens practice and to implement improvements in the design of the corneal lens.

In this episode BCLA commitee members, Fakhruddin and Karin, interview Dr Eef van der Worp about the contents of the paper. Eef is an educator and researcher. He runs his own research & education consultancy 'Eye-Contact-Lens' based in Amsterdam (NL). He is on the education committee for a number of international conferences, including the Global Specialty Lens Symposium (GSLS) in the US and the Dutch Contact Lens Conference (NCC) and Dutch Optometric Society meeting (OVN). He is a board member of the BCLA journal Contact Lens & Anterior Eye.

In the tenth and final instalment in the BCLA CLEAR podcast series Dr Melissa Barnett – a Fellow of the BCLA and Global Ambassador for the BCLA – interviews Dr Laura Downie about the BCLA CLEAR paper on anatomy and physiology of the anterior eye. A key element of contact lens practice involves clinical evaluation of anterior eye health, including the cornea and limbus, conjunctiva and sclera, eyelids and eyelashes, lacrimal system and tear film. This report reviews the fundamental anatomy and physiology of these structures, including the vascular supply, venous drainage, lymphatic drainage, sensory innervation, physiology and function. This is the foundation for considering the potential interactions with, and effects of, contact lens wear on the anterior eye. This information is not consistently published as academic research and this report provides a synthesis from all available sources. With respect to terminology, the report aims to promote the consistent use of nomenclature in the field, and generally adopts anatomical terms recommended by the Federative Committee for Anatomical Terminology. Techniques for the examination of the ocular surface are also discussed.

Dr Downie is an Associate Professor in the Department of Optometry and Vision Sciences at the University of Melbourne, where she heads the ‘Anterior Eye, Clinical Trials and Research Translation Unit’ comprising 13 members. Laura’s research combines laboratory, clinical and implementation science as a foundation for improving patient outcomes, with a focus on the anterior eye.

In the ninth instalment in the BCLA CLEAR podcast series Dr Melissa Barnett – a Fellow of the BCLA and Global Ambassador for the BCLA – interviews Dr Daddi Fadel about the BCLA CLEAR paper on orthokeratology. Orthokeratology (ortho-k) is the process of deliberately reshaping the anterior cornea by utilising specialty contact lenses to temporarily and reversibly reduce refractive error after lens removal. Modern ortho-k utilises reverse geometry lens designs, made with highly oxygen permeable rigid materials, worn overnight to reshape the anterior cornea and provide temporary correction of refractive error. More recently, ortho-k has been extensively used to slow the progression of myopia in children. Melissa and Daddi discuss just some of this in the episode.

The full report reviews the practice of ortho-k, including its history, mechanisms of refractive and ocular changes, current use in the correction of myopia, astigmatism, hyperopia, and presbyopia, and standard of care. Suitable candidates for ortho-k are described, along with the fitting process, factors impacting success, and the potential options for using newer lens designs. Ocular changes associated with ortho-k, such as alterations in corneal thickness, development of microcysts, pigmented arcs, and fibrillary lines are reviewed. The safety of ortho-k is extensively reviewed, along with an overview of non-compliant behaviours and appropriate disinfection regimens. Finally, the role of ortho-k in myopia management for children is discussed in terms of efficacy, safety, and potential mechanisms of myopia control, including the impact of factors such as initial fitting age, baseline refractive error, the role of peripheral defocus, higher order aberrations, pupil size, and treatment zone size.

Daddi Fadel DOptom, FSLS, FBCLA, FAAO, is a contact lens designer, a Pioneer of modern lens designs, and a specialist in contact lenses for irregular cornea, scleral lenses, myopia control, and orthokeratology. She has 20 years’ experience in optometry and specialty contact lenses. Daddi runs an optometric practice specializing in contact lenses in Italy where she personally designs and fits special customized contact lenses. She is internationally recognized as a key opinion leader, who has several peer-reviewed publications in the area of specialty contact lenses and is a speaker in national and international symposia, and also holds fellowships with numerous institutions.

In the eighth instalment in the BCLA CLEAR podcast series Dr Melissa Barnett – a Fellow of the BCLA and Global Ambassador for the BCLA – interviews Dr Fabrizio Zeri about the BCLA CLEAR paper on contact lens materials. Dr Zeri is a Professor at the University of Milano-Bicocca and Visiting Research Fellow at Aston University in Birmingham. Contact lens materials have undergone significant changes over the past 20 years, particularly with respect to the introduction of silicone hydrogel materials. Whilst this development addressed hypoxic issues, other important areas relating to contact lens success, notably comfort, require further research. Contact lens wettability remains a crucially important part of biocompatibility. Dr Barnet and Dr Zeri cover some of this and more in this episode. Contact lenses can be made more wettable by incorporation of surfactants into blister packs, internal wetting agents, surface treatments or care solutions. However, there remains no clear association between contact lens wettability and comfort, making it challenging to determine the potential for these approaches to be of significant clinical benefit. Most contact lenses are used on a daily wear, reusable basis, which requires them to be disinfected when not worn. The ideal disinfecting solution would also improve comfort during wear. However, balancing these requirements with other factors, including biocompatibility, remains a challenge. Soft lens materials invariably take up and subsequently release certain components of disinfecting solutions onto the ocular surface. This may affect tear film stability and the normal ocular microbiome, and further research is needed in this area to determine whether this has any affect on comfort. Finally, contact lens materials sorb components of the tear film, and these interactions are complex and may change the biochemistry of the tear film, which in turn may affect their comfort.

In the seventh instalment in the BCLA CLEAR podcast series Dr Melissa Barnett – a Fellow of the BCLA and Global Ambassador for the BCLA – interviews Professor Lyndon Jones - Director of CORE and also a Fellow of the BCLA - about just some of the future applications of contact lenses and what the profession and society can look forward to. Contact lenses in the future will likely have functions other than correction of refractive error. Lenses designed to control the development of myopia are already commercially available. Contact lenses as drug delivery devices and powered through advancements in nanotechnology will open up further opportunities for unique uses of contact lenses.

This review examines the use, or potential use, of contact lenses aside from their role to correct refractive error. Contact lenses can be used to detect systemic and ocular surface diseases, treat and manage various ocular conditions and as devices that can correct presbyopia, control the development of myopia or be used for augmented vision. There is also discussion of new developments in contact lens packaging and storage cases.

In this episode Dr Melissa Barnett – a Fellow of the BCLA and Global Ambassador for the BCLA – interviews Dr Deborah Jacobs about medical use of contact lenses which is a solution for many complex ocular conditions, including high refractive error, irregular astigmatism, primary and secondary corneal ectasia, disfiguring disease, and ocular surface disease. The development of highly oxygen permeable soft and rigid materials has extended the suitability of contact lenses for such applications. There is consistent evidence that bandage soft contact lenses, particularly silicone hydrogel lenses, improve epithelial healing and reduce pain in persistent epithelial defects, after trauma or surgery, and in corneal dystrophies.

Drug delivery applications of contact lens hold promise for improving topical therapy. Modern scleral lens practice has achieved great success for both visual rehabilitation and therapeutic applications, including those requiring retention of a tear reservoir or protection from an adverse environment. This report offers a practical and relevant summary of the current evidence for the medical use of contact lenses for all eye care professionals including optometrists, ophthalmologists, opticians, and orthoptists. Topics covered include indications for use in both acute and chronic conditions, lens selection, patient selection, wear and care regimens, and recommended aftercare schedules. Prevention, presentation, and management of complications of medical use are reviewed.

This is the fifth episode in the BCLA CLEAR series. Scleral lenses were the first type of contact lens to restore vision and protect the ocular surface. With the advent of rigid corneal lenses in the middle of the twentieth century and soft lenses in the 1970’s, the use of scleral lenses diminished; in recent times there has been a resurgence in their use driven by advances in manufacturing and ocular imaging technology. Scleral lenses are often the only viable form of contact lens wear across a range of clinical indications and can potentially delay the need for corneal surgery. This report provides a brief historical review of scleral lenses and a detailed account of contemporary scleral lens practice including common indications and recommended terminology. Recent research on ocular surface shape is presented, in addition to a comprehensive account of modern scleral lens fitting and on-eye evaluation. A range of optical and physiological challenges associated with scleral lenses are presented, including options for the clinical management of a range of ocular conditions. Future applications which take advantage of the stability of scleral lenses are also discussed. In summary, this report presents evidence-based recommendations to optimise patient outcomes in modern scleral lens practice.

This is the fourth episode in the BCLA CLEAR series. By reviewing the physiological and anatomical changes which may occur while wearing contact lenses this paper provides eye care practitioners with a better understanding of the key clinical learnings for modern contact lens practice. This report consolidates clinical research information about the various physiological and anatomical changes which may result from using contact lenses. It takes an anatomy-centric approach covering changes to the lids and adnexa, the conjunctival regions and the various layers of the cornea and presents changes which are sub-clinical rather than those which are symptomatic or generally considered as 'adverse events'.

Professor Phil Morgan discusses some of the key findings of this paper with Dr Melissa Barnett. Professor Morgan is Professor of Optometry, Head of Optometry, Deputy Head of the Division of Pharmacy and Optometry, and Director of Eurolens Research at The University of Manchester.  His main research interests relate to the clinical performance of contact lenses and he teaches on the same subject area at undergraduate and postgraduate level.  He has managed an international survey of contact lens prescribing trends since 1996. He is also President of the International Society for Contact Lens Research and President of the International Association of Contact Lens Educators. He is an honorary member of the UK Association of Contact Lens Manufacturers, a Fellow of the British Contact Lens Association and the American Academy of Optometry.

This is the third episode within the BCLA CLEAR podcast series. The most fundamental aspect of a contact lens is its optics; the manner in which the refraction of light is managed to optimise vision to the clinical benefit of the lens wearer. This report presents contemporary information on the optical structure of the eye and the optical models employed to understand the correction of refractive error. The design, measurement and clinical assessment of spherical, aspheric, toric, multifocal and myopia control contact lenses are described. The complexity and variety of multifocal lenses is recognised and detailed information is provided for alternating, simultaneous, diffractive, annular, aspheric and extended depth of field lens designs. In terms of clinical assessment, a contemporary review is provided for the measurement of: visual acuity, contrast sensitivity, through focus curves, reading performance, peripheral refraction, toric displacement realignment and patient reported outcomes. Overall, the paper aims to serve as a resource for the prescribing clinician, who can optimise contact lens corrections for patients by building on the optical rationale of these devices; and also highlights future opportunities for research innovation.

Dr Kathryn Richdale is interviewed by Dr Melissa Barnett, who leads the BCLA CLEAR podcast series. Dr. Richdale is currently an attending in the Cornea and Contact Lens Service and co-director of the Myopia Management Service, and teaches in both the optometric and graduate programs at UHCO. She conducts research in the areas of cornea, contact lenses and refractive error.

This has required new ways of working and Rebecca will be highlighting how they have used some of this change to continue delivering care to her patients. Rebecca is a therapeutic optometrist and currently works as the clinical lead for a group of independents in the Bristol area. She has experience across independent, multiple and hospital services in both Ireland and the UK. She received her undergraduate honours degree from Technological University, Dublin and has a special interest in contact lenses, dry eye management and glaucoma.

This is the second episode in the BCLA CLEAR podcast series. While severe complications with contact lenses are rare, the majority can be systematically identified, managed and prevented in practice. The paper on contact lens complications is part of the BCLA Contact Lens Evidence-based Academic Reports (CLEAR) – a global consensus report put together by the BCLA. This report summarises the best available evidence for the classification, epidemiology, pathophysiology and management of contact lens complications. Complications are considered from an aetiological perspective to help practitioners to determine the most appropriate management strategy and key signs and symptoms underlying major groups of complications are described to support the decision- making process.

In this podcast series we will focus on separate reports, highlighting key findings and other areas of interest.

This episode features, Professor James Wolffsohn, who will discuss the work in the report on evidence based contact lens practice. This report reviews and summarises the published research evidence that is available to inform soft and rigid contact lens history and symptoms taking, anterior eye health examination (including the optimised use of ophthalmic dyes, grading scales, imaging techniques and lid eversion), considerations for contact lens selection (including the ocular surface measurements required to select the most appropriate lens parameter, lens modality and material selection), evaluation of lens fit, prescribing (teaching self-application and removal, adaptation, care regimen and cleaning instructions, as well as minimising risks of lens wear through encouraging compliance) and an aftercare routine. 

Professor Wolffsohn is interviewed by Dr Melissa Barnett, who leads the BCLA CLEAR podcast series. Dr. Barnett is a principal optometrist at the University of California, Davis Eye Center in Sacramento and Davis, California. She is an internationally recognized key opinion leader, specializing in dry eye disease and specialty contact lenses. She is a Fellow of the American Academy of Optometry, a Fellow of the British Contact Lens Association and serves on multiple Boards. Dr. Barnett is Past President of The Scleral Lens Education Society.

Josie Barlow, Professional Services Manager at Menicon, is joined by Jonathan Jackson, Head of Optometry at the Belfast Health and Social Care Trust, to discuss keratoconus and some of the aspects of managing this eye condition. Josie and Jonathan touch on the treatments available, the importance of teamwork, and a few tips on how to manage keratoconus effectively. The podcast serves as a taster for the webinar where Jonathan and his team will discuss further details about how they manage treatment at his busy hospital. Ahead of their upcoming webinar, 'The “case” for water avoidance in contact lens wear', Dr Nicole Carnt and Irenie Ekkeshis discuss their work that led to a greater public awareness of water-related infection associated with contact lens wear - specifically acanthamoeba keratitis. Nicole and Irenie were both pivotal in making clearer patient information available as well as campaigning for no water stickers on contact lens cases. 

Dr Carnt is currently a Scientia Fellow at UNSW, Sydney, and Chair of the Australian Standards Contact Lens Committee and Expert to International Standards Organization. Irenie is an acanthamoeba keratitis patient leader and her efforts to raise awareness of this infection and the 'no water' campaign have been recognised through the award of campaign of the year at the RNIB vision pioneer awards in 2016.

In 2020 Johnson and Johnson Vision launched their Prioritise Your Eyes campaign. This initiative was aimed at raising awareness of the importance of having regular eye tests. They carried out research that canvassed opinions from the public in many different countries to understand their perceptions of eye health. In this episode Rachel Hiscox, Professional Education and Development Manager at Johnson and Johnson Vision, provides insights into some of the key findings of the research. We are also joined by Gary, a patient and advocate for eye tests. Gary shares his own tale about having an eye examination that revealed a deeper underlying issue.

Ankur Barua is a consultant ophthalmologist specialising in complex corneal, refractive and cataract surgery based in Birmingham Midland Eye Centre (BMEC).

Rakesh Kapoor is a qualified Optometrist with just under 30 years' experience within the Optical Industry. Shortly after qualifying he opened Specsavers Opticians Wembley in 1994. Subsequently he now runs 3 stores in North West London.

Both Ankur and Rakesh are both BCLA Council members. 

Each year brings challenges in a number of forms. When providing care for a patient they can present challenges too: complications and conditions that require extra attention. But what about patients' wellbeing? They may present specific afflictions to their eye health, but care can sometimes go beyond that. Eye care professionals are an important part of patients' holistic wellbeing and it may be important to ask them how they are. This episode features Neil Retallic and Sheena Shah, a second one focussing on wellbeing. 

Sheena is an optometrist working in Vision Express. She released her first book in 2020 on wellbeing and runs two businesses that are not eye related. A key professional interest of hers is wellbeing in the eyecare profession and she leads the discussion in this episode.

Effectively diagnosing and managing dry eye disease has become an increasingly important distinct part of eye care services, but not enough ECPs know about it or are confident enough to manage it effectively. There are growing numbers of patients who are experiencing the discomfort that this brings and need our help. This episode introduces dry eye disease and aims to improve your basic understanding and how to build on this. It references findings from the TFOS DEWS II report and conference, as well as information about the BCLA's dry eye course.

Dr Byki Huntjens is a senior lecturer at City, University of London. She is a qualified optometrist with a Masters and Doctorate at the University of Manchester. Her current research interests include contact lenses and dry eye. Byki is a BCLA council member, holds fellowships from the BCLA, IACLE and the Higher Education Academy, and is vice-chair of BUCCLE.

Sarah Farrant is an independent prescribing optometrist and practice owner based in Earlam and Christopher Optometrists, an independent practice in Taunton providing eye care, eye wear and specialist clinics. She has strong interests in the ocular surface and dry eye disease - seen as an expert by many and regularly contributes on the topic.

Whether it is generally trying to get work/life balance right, or dealing with day-to-day stresses, ensuring we are mentally, emotionally and physically healthy should be a central consideration. In this podcast current BCLA President, Jonathon Bench, and President Elect, Indie Grewal, discuss their thoughts on their own Presidential address topics and what life has been like in and for the BCLA over the last 12 months or so.  A period of great change for the profession, the contact lens sector and for the BCLA too.

An optometrist by trade, Jonathon has worked as resident and mobile optometrist in both multiple and independent practices, and been a joint venture partner, before moving into contact lens category management. He now works as Head of Professional Affairs for Alcon where he looks after Alcon Academy and in-field professional development teams; delivering training, support and education to eye care practitioners throughout the UK and Ireland.

Indie is a qualified Dispensing Optician and Optometrist with 25 years experience within the Optical Industry. He opened his own Leightons Franchise in St Albans in 2001. As well as participating in contact lens clinical trials he has a special interest in Myopia Management and Multi-focal contact lens fitting. Indie’s passion for contact lenses has seen him work with major contact lens manufacturers and as a key opinion leader he has presented both in the UK and internationally

PPE has always been part of health care, but during the coronavirus pandemic there has been an increased expectation to increase this use. For many this includes using new PPE items and adapting to it whilst carrying out procedures. Join Luke Allen and Nick Howard as they discuss their experience of wearing PPE and carrying out eye care services using PPE. 

Nick Howard qualified as a Contact Lens Optician in 1984, working primarily with challenging ocular conditions and complex contact lenses. He is a part time lecturer, trainer and presenter and facilitator at conferences, hospitals and universities at local, national and international events, also author of a number of articles published in the professional optical journals. Winner of prestigious Contact Lens Optician of the Year award in 2012, awarded Fellowship of the BCLA and currently a BCLA Faculty speaker. He works in two Lancashire hospitals fitting specialty contact lenses, also independent practice, developing innovative techniques for management of myopia and dry eye.

Luke Allen graduated from Cardiff University with first class honours in 2014 and qualified as an Optometrist in 2015. At university he had a special interest in contact lenses and wrote his dissertation on multifocals. Carrying this forward to work in private practice he launched the offering of Ortho-keratology and semi-scleral contact lenses in a previous practice. Keen on myopia management, he was also one of the first practitioners in the UK to start fitting MiSight© 1 day contact lenses and NaturalVue contact lenses. Currently working in private independent practice managing and treating more severe dry eye cases, as well as fitting specialist contact lenses.

It has been a tumultuous time for the world and medical professions have been particularly impacted. As restrictions are eased across a number of countries and the return to full services imminent the BCLA held a survey to understand the emotions and considerations for those practising within these specialised areas of eye care. The results revealed a number of trends within the wider community, particularly thoughts on the future and the role that technology has played and will continue to play in the future.
